I agree that by submitting my card information, UAD will charge my card every year to renew my membership automatically until I inform UAD that I want to cancel my membership via treasurer@uad.org or other means.

I understand that if I initiate a chargeback of membership fees and it turns out that I forgot I authorized this charge, UAD may choose to charge me a $30 fee.

If I choose to pay via paper check and my check is returned for insufficient funds, UAD may choose to charge me a $30 fee.
