To be considered for continued authorization by the Utah Association for the Deaf, Inc. to purchase airline tickets for official UAD business and to be reimbursed by the UAD, I hereby agree to be governed by the following provisions:

1. The flight must be reserved 21 days or more in advance, or as instructed by the president or the treasurer.
2. Attendance at all business meetings of the event is required.  Saturday night stayover is required.
3. If I purchase an airline ticket in order to travel on behalf of UAD and I am reimbursed by the UAD, and for any reason I am unable to attend the event, I agree to assume financial responsibility for the repayment of the penalty value of the ticket.
4. If I purchase an airline ticket in order to travel on behalf of UAD and I am reimbursed by the UAD, and I do not use the ticket because of extenuating circumstances (such as medical or family emergencies) which prevent me from using the ticket, I agree to notify both the president or the treasurer and the travel agency of my inability to attend the event.  I further agree to abide by the instructions of the travel agency regarding the return of tickets.
5. Failure to notify the president or the treasurer and the travel agency or to personally compensate UAD for the penalty value of the ticket will result in the suspension of authorization to purchase tickets on behalf of UAD, until full reimbursement is made to the UAD.  In addition, UAD reserves the right to suspend me from all activities if I fail to comply with the above policies.
